Eight Disciplined (8D) Problem Solving is a method used to approach and to resolve problems. The development of a Team Oriented Problem Solving (TOPS) strategy, based on the use of statistical methods of data analysis, was pioneered at Ford Motor Company in 1986 resulting in a method called TOPS 8D. A revised version called "Global 8D" (G8D), the current global standard for many companies in the automotive supply chain, was developed in the late 1990s and since then, 8D has migrated across many industries and become regarded as the most complete problem solving method available.
When applied thoroughly, 8D will lead to the discovery of root cause(s) and potential solutions whilst considering costs, timing, effect on customers and impact on the organisation.
Considering Human Factors is crucial to root cause analysis as ‘operator error’ alone can never be considered root cause when error proofing or improved process capability will permanently reduce or eliminate recurrence of a problem.
